Audition Request Form

Please use the contents of this form to request a personal meeting or audition or simply write to us and provide the contact and description information that you are willing to provide. Please send the information to our e-mail address or to our postal mailing address. Meetings are held at a mutually agreed public place due to liability issues. Please do not come to our location, you will not be assisted there. The address provided at our web sites is for mailing purposes only. All mail received there is forwarded to another undisclosed location to ensure that meetings are not held at private locations (again, for liability reasons).

Minors must ask permission from parent(s) or guardian(s) prior to sending us private information such as the information below. 

If the person having the audition is a minor then an adult responsible for the minor must be present at the time of the audition and at all of the meetings held with us. Otherwise the meetings will be cancelled.
